Effect of Implementing Nursing guidelines regarding Drug overdose poisoning among Critically ill patient`s outcomes

Background: Drug overdose poisoning is a common cause admission at medical emergency unite. With chief complaint resulting many complications as central nervous system, hypothermia, respiratory depression, hypotension. Aim of the study: This study was carried out to evaluate the effect of Implementing Nursing guidelines regarding drug overdose poisoning among critically ill patients at Medical Emergency Unite at Assiut University Hospitals. Subjects and Methods: Research design: a quasi-experimental design. Setting: Medical emergency unite at Assiut University Hospitals and the study started from July 2016 till July 2018. Subjects: A convenience sample of 60 adults' critically ill patients diagnosed with drug overdose poisoning. The patients were assigned into two equal groups (control and study group).Tools: two tools were utilized to collect data, Tool I: Patient's assessment questioner that consists of three parts: part one: assessment of the Patients profile, part two: drug overdose data questioner, part three: biomedical data and level of conscious. Tool II: patients outcomes questioner that consist of part one: Laboratory tests findings, part two: hospital stay and progress part three: complications of poisoning) Methods: The research done from first day of admission for twenty four hours and every shift during this phase the patients received the nursing guidelines, this was done by comparing the results of outcomes for the both groups by using tool two. Results: (34%-26.7%) from age groups (30-40) years in the both groups. The highest percentage of the two groups ( 36.7% and 50%) were males. There was hypotension in control group with means of (95±11.37- 59.2±6.47) respectively. But in study group, normal with mean (99.9±10.28- 65.63±5.86 ) respectively. There were highly statistical significant differences between both groups with p value = (0.005**). ( 36.3 -30%) in the both groups had experienced metabolic acidosis Conclusion: The study group who implemented nursing guidelines was found to experience lesser complications than control group. Recommendations: Developing management and apply clinical practice guidelines for optimal patient care with drug overdose poisoning.

Effect of Parent's Counseling Sessions on the Modification of Aggressive Behavior among Their School Age Children

Background: Aggressive behaviors in school – age children are very common. Aggression is the most serious inappropriate behavior that has the most serious consequences for both the child and environment. Aim of the study: The study aimed to assess the effect of parent's counseling session on modifying the aggressive behavior among their school age children. Subjects and Methods: Research design: Quasi-experimental design was used in this study. Setting: The study was conducted at two school involved in governmental and private school at Zagazig city. Subjects: A purposive sample of 120 children and their parents (60 from each school) divided equally to study group and control group. Tools of data collection: Three tools were used for data collection. Tool (I): A questionnaire interview sheet. Tool (II): Aggressive behavior scale and Tool (III): Adult– Adolescent Parenting Inventory. Counseling sessions for parents were developed by the researcher. Results: More than half of the studied children of both school had aggressive behavior and problems in their families before intervention compared with the control group. There was a statistically significant difference regarding total aggression and total parents attitudes score throughout session's phases. Conclusion: It can be concluded that the counseling sessions had improved the studied parents' knowledge and attitudes about modification of aggressive behavior of their school age children. Recommendations: Providing continuous counseling sessions for parents of aggressive children about behavioral modification, appropriate parenting practices and scientifically related books to be available for all parents in order to follow up is essential.

Relation between Conflict and Perception of Professionalism among Nurses working at Kafr Sakr General Hospital

Background: Nurses executives and middle managers can create work atmospheres free of conflict, in which staff nurses perceive opportunities to increase their professionalism and improve professional practice to perform these functions, managers need to understand the nature of conflict and the definition of professionalism. Aim of the study: To explore the relation between conflict and perception of professionalism among nurses. Subject and Methods: Research Design: Descriptive correlational design was utilized in this study. Setting: The study was conducted at Kafr Sakr General Hospital. Subjects: A convenience sample of nurses (n=184). Tools of data collection: Two tools used for collect data namely: nursing conflict scale and valiga concept of nursing scale. Results: revealed that the most common types of conflict was intrapersonal followed by disruptive and the lowest type was intergroup (86.9%, 82.2%& 69% respectively). The highest percentage of nurses had high level of conflict (79.6%). Also, the highest percentage of nurses had a higher perception of professionalism (81.5%). Conclusion: There were statistically significance relation between nurse's perception toward professionalism and level of all conflict types (p=0.01). Recommendations: Efforts should be made by the nurse managers to occasionally stimulate conflict by promoting different views and motivating staff and unit/department through rewards for their outstanding performance. The subject of professional nursing should be included in the curriculum of nursing education.

Nurses Knowledge and Practice Regarding Care of Comatose Children at Pediatric Intensive Care Units

Background: Coma is a state of prolonged unconsciousness in which the patient cannot be aroused even with painful stimuli. Coma in children is categorized into traumatic and non- traumatic (NTC). Aim of the study: was to assess nurses' knowledge and practice regarding care of comatose children at pediatric intensive care units. Subjects& Method: Research design: A descriptive design was used. Setting: The study was conducted at Pediatric intensive care unit in Pediatric hospital at Zagazig University Hospitals, Pediatric intensive care unit at Al Ahrar Hospital and Pediatric intensive care unit at General Hospital. Subjects: The study sample included all accessible nurses who work at the previous setting. Tools of data collection: Two tools were used to collect the necessary data. The first was a structured interview questionnaire. The second was observational checklist for nurses' practice. Results: The study results revealed that the total scores of nurses' knowledge were good, fair and poor with percentage 69.2, % 27.7% and 3.1% respectively while the total practices level of all studied nurses were unsatisfactory. Conclusion: The study concluded that more than two third of studied nurses had good knowledge regarding care of comatose children while all studied nurses' practice provided to comatose children was unsatisfactory. Therefore, the expected care provided to comatose children was unsatisfactory. Recommendations: The study recommended that continuous formal training programs about care of comatose children will help nurses to update their knowledge and improve their practice was recommended.

Nephrotic Children' Perspectives of Parental Practices and Their Relation with Quality of Life and Drug Adherence

Background: Nephrotic syndrome is the most frequent chronic renal disorder in children that is
associated with high relapse rate. . Aim of the study: to assess nephrotic children' perspectives of parental practices and their relation with quality of life and drug adherence. . Subjects& Method: Research design: A descriptive study was used. Setting: The study was conducted at Pediatric Urology Outpatient Clinic at Zagazig University Hospitals. Subjects: The study sample included100 children with nephrotic syndrome. . Tools of data collection: Five tools were used astructured interview questionnaire to collect data about personal characteristics of studied children, parenting style scale, modified parental support scale, pediatric quality of life inventory and morisky 8-item medication adherence scale. Results: The study results revealed that 64% of children were male with mean age of 10 , 73% were from rural. 72% of parents were authoritative. 60 % of children had highly supportive score. 83% of children had good QOL score. 45% had moderate drug adherence score. Conclusion: The study concluded that there was highly statistically significant relation (p < 0.001) between parenting style, parental support, child QOL and drug adherence. Recommendations: Based on the results of the present study, it can be recommended that orientation programs for children and parent about disease, Qol and drug adherence.

Work Stress as perceived by nurse interns at Zagazig University hospitals

Background Nurse Interns is considered stressful for nursing staff at all levels. They need to exhibit more efforts to face the demand of that profession. Physical symptoms of work-related stress are common. Aim of the study: was conducted to assess stress as perceived by nurse interns at zagazig university hospitals. Subjects and Methods: Research design: A Descriptive design was employed to convey the present study. Setting: The study was conducted in the different clinical areas of Zagazig University Hospitals where the nurse interns practice their training in the academic year 2017- 2018. Subjects: included all nurse interns (n=118). Tools of data collection Perceived Stress Scale was used to collect the data. Results: All nurse interns (100 %) had high level of stress; there is a statistically significant difference between the total mean score of stress among nurse interns. Also, there is a statistically significant relation between nurse intern's stress score and all personal characteristics except marital status. Conclusion: all nurse interns (100 %) had high level of stress, and the highest mean score of stress was related to lack of knowledge and skills, while the lowest mean score was related to clinical environment. Recommendation: that design & implement stress management programs for nursing students. It should aim to educate students nurse with better stress management skill& faculty members should emphasize participation in stress coping programs to improve academic performance of nurse students.

Background: Diabetes mellitus metabolism disorder characterized by chronic hyperglycemia with disturbance of carbohydrate, fat and protein metabolism resulting from defects in insulin secretion ,insulin action ,or both leading to variety of medical complication such as hypoglycemia . Aim of the study: assess depression and health related quality of life in diabetic adolescents at Zagazig city subjects and methods: Research design: was analytical case- control design. Setting was conducted at Diabetes outpatient clinic at Zagazig University Hospital, Diabetes clinic at Almabra Hospital, Preparatory and Secondary school at Zagazig city. Subjects 100 of diabetic adolescent aged 12-18 years and 100 of control group who attended the previous settings. Tools of data collection: Three tools were used for data collection: Tool I: questionnaire interview sheet and Tool II was depression inventory scale The Tool III: was Pediatric Quality of life inventory. Results: revealed that half of diabetic adolescents had depression compared to control group. Two thirds of diabetic adolescents had poor quality of life compared to control group. Conclusion: The study findings concluded that diabetes effect on quality of life In diabetic adolescents, There were relation between depression and health related quality of life in studied group. Recommendations: This study recommended that design training guiding programs for diabetic adolescent about blood glucose test ,sites of insulin injection ,risk of complication , diet restrain .orientation of the studied group about symptoms of hypo and hyperglycemia and how control it

Factors Influencing Nurses ' Compliance with Standard Precautions Regarding Occupational Exposures to Blood and Body Fluids

Background: Standard Precautions(SP) are the minimum infection prevention practices that apply to all patient care, regardless of suspected or confirmed infection status of the patient. Aim of the study : Was to assess factors that influence nurses' compliance with standard precautions regarding occupational exposures to blood and body fluids. Subjects and Method: Research Design: A descriptive exploratory study design was used. Setting: The study was conducted at Faqous General Hospital in Sharkia governorate, Egypt. Subject: A Convenience sample of 86 nurses. Tools of data collection: Two tools were used Tool I. A self-administered questionnaire composed of four part: part 1: Concerned with nurses' demographic data. part 2: Covered nurses' occupational exposures rate to blood and body fluids, part 3:Revealed nurses' compliance with standard precaution and part 4: Covered the factors influencing nurses' compliance with (SP) included ,the hospital factors, nurses' attitude and nurses' knowledge. Tool II: Observational checklist to assess nurses' practice. Results: The study finding revealed that the nurses median age was 30.0 years,76.7% were females,68.6% of the sample had previous occupational exposure , 87.2% of the nurses were compliant with standard precaution, 68.6% of the nurses had satisfactory knowledge on standard precautions ,75.6% had satisfactory practice ,Additionally there were a significant positive correlations between nurses compliance scores on standard precaution and hospital related factors(r = 0.267). Conclusion : Hospital staff nurses are at a significant risk of exposure to blood and body fluids. The majority of the nurses were compliant with standard precaution. While, more than two third of nurses had satisfactory total knowledge regarding (SPs) Recommendations: Maximizing the hospital role in updating the staff 'knowledge about standard precautions through empowering an infection control committee .

Effect of Education Program on Nurses Performance Regarding Traumatized Patient Care during the Golden Hour in Emergency Room at Zagazig University Hospital

Background: Trauma is the leading cause of mortality in young adults. Many such deaths are preventable if patients are treated effectively. The term "golden hour" is commonly used to characterize the urgent need for the care of trauma patients. This study aimed to evaluate the effect of implementing an educational program on nurses' performance regarding Traumatized patient during the Golden hour in emergency room at Zagazig University Hospital. Subjects and Methods: Research design A quasi experimental design was utilized. Setting The study was conducted at the emergency departments of Emergency Hospital at Zagazig University. Subjects: Fifty Emergency nurses. Tools of data collection Four tools were used for collection of data, First questionnaire to collect demographic characteristics of study nurses and questions to assess nurses' knowledge regarding trauma patients during the golden hour of care. Second tool was an observational checklist to assess nurses' practice. Third tool to assess the nurse's attitude. Four tool to assess the opinion of nurses. Results there was statistical significant difference in study phases were found in nurses' knowledge and their practices throughout program phases. Total nurses knowledge in pre-program (72%) unsatisfactory as well as in post-program (100%) satisfactory, and follow up (88%)satisfactory, Total nurses practices in pre-program phase(100%) unsatisfactory, as well as in post-program (100%) satisfactory and follow up (78%) satisfactory, Total nurses attitude in pre/post and follow up phases (100%) satisfactory. Conclusion: The Education Program had Positive effect on nurses performance regarding Traumatized Patient care during golden hour, there were statistical significant differences between nurses knowledge and practices regarding to traumatized patient during golden hour in emergency department after program implementation either immediately or three month later. Recommended: Development of Training Program should be conducted periodically for nursing staff in EM; Hospitals should develop evidence based guidelines on Traumatized patient.

Back ground: The need for critical thinking in nursing has been accentuated in response to the rapidly changing health care environment .Nurse must think critically to provide effective care whilst coping with the expansion in role associated with the complexities of current health care system. The aim of the present study was conducted to assess the critical thinking on developing & motivation to learn among nursing students. Subject & method: Research design:, Descriptive correlation design was used to conduct the present study: Setting: the current study was conducted at faculty of nursing, zagazig university. Subject of the study will include all nursing students enrolled in the academic year 2017-2018. Tools of data collections: Tool I: Nurse Student's knowledge assessment questionnaire to assess nurse student's knowledge about critical thinking, Tool II: California critical thinking skills test it was to assess the nurse student's skills& Tool III: Motivation to learn questionnaire. Results the present study reveals there's statistically significant between critical thinking, and motivation to learn. Conclusion: it could be concluded that: all nursing student had inadequate level of knowledge regarding critical thinking, the highest percentages of them had a weak level of critical thinking skills and a moderate level of motivation to learn. Recommendations: Development of curriculum contents, which concentrates on improving thinking skills-in general- and critical thinking skills-in specific.

Effect of professional nursing practice environment and Psychological empowerment on nurses' readiness for change

Background: The nursing shortage is an international problem and improving the practice environment has been shown to be a successful strategy against this phenomenon. Psychological empowerment of nurses and supportive practice environment is significantly associated with higher job satisfaction, job retention, quality of care and stimulating and managing organizational change. The Aim of the study: to assess the effect of a professional nursing practice environment and psychological empowerment on nurses' readiness for change. Subjects and Methods: Research design: A descriptive correlational design was used. Setting: the study was conducted at New Surgery Hospital in Zagazig University Hospitals. Subjects: A convenient sample of the nurses (n= 300). Tools of data collection: Three tools were used for data collection: Readiness for change scale; practice environment scale of the nursing work index, and nurses' psychological empowerment questionnaire. Results: the highest percentage of nurses had a high level of readiness for change and psychological empowerment (77%& 65%, respectively). The majority of the nurses (79%) had a moderate level of professional nursing practice environment. Conclusion: There was a positive statistically significant correlation between readiness for change and professional nursing practice environment scores. Also, there was a positive statistically significant correlation between professional nursing practice environment and psychological empowerment scores. Recommendations: Nurse Managers should encourage staff nurses to participate in making decision and listen to nurses concerns, Nurse Managers should conduct in-service training program about change that can support nurses to be adapted with change, and reduce emotional exhaustion and Nurse Managers should develop new ways to involve staff nurses in clinical decision making.
